ABSTRACT

Objective@#To study the effect of continuous nursing care on quality of life in patients with rheumatoid arthritis after discharge.@*Methods@#Patients with rheumatoid arthritis who were admitted to hospital from August 2016 to August 2017 were selected as the study subjects. According to the random number table method, they were divided into the control group and the research group, and the lost cases were eliminated. There were 99 cases in the control group and 94 cases in the research group. The control group was given routine care, and the research group conducted continuous nursing care measures on the basis of the control group. The morning stiffness time, pain score, disease activity, patient health status and nursing satisfaction of the two groups were observed before and 6 weeks after discharge.@*Results@#Six weeks after discharge, the morning stiffness time (23.76±12.76) min of the research group was significantly higher than that of the control group (38.22±14.53) min, the difference was statistically significant (t=3.865, P<0.05). Six weeks after discharge, the VAS (2.01±0.68) and HAQ (0.42±0.34) points of the research group were lower than those of the control group (4.17±1.30), (0.92±0.63) points, the difference was statistically significant (t=4.073, 3.916, P<0.05). The nursing satisfaction of the research group was 94.68%(89/94), and that of the control group was 78.79%(78/99), and the difference was statistically significant (χ2=9.416, P < 0.05).@*Conclusions@#The implementation of continuous care mode after discharge from patients with rheumatoid arthritis is beneficial to alleviate clinical symptoms such as morning stiffness and pain. Moreover, it can also improve patient health and quality of life, increase risk of disease progression, and care satisfaction is high which can help to protect the harmony between nurses and patients.

ABSTRACT

Objective To compare the efficacy of acrossing the injured vertebra percutaneous pedicle screw fixation and traditional open surgery for Magerl A thoracolumbar fractures.Methods Retrospective analysis 56 patients with single segmental Magerl A thoracolumbar fractures without neurological symptoms were stabilized,including percutaneous fixation in 26 cases and open fixation in 30 cases.The operation time,intraoperative,blood loss,incision length,sagittal Cobb angle,height of injured vertebral,VAS,ODI,were compared between 2 groups.Results The precutaneous group less surgical blood loss,incision length and postoperative go to the fields time than the open group (P < 0.05).There were improvements in Cobb angle and anterior height in two groups (P < 0.05).The precutaneous group had lower VAS and ODI scores postoperation (P < 0.05).The fracture vertebral front height and Cobb angle had significantly loss at final follow up than on the third day postoperation (P < 0.05).Conclusions The percutaneous pedicle screw fixation can achieve similar effect in the treatment of Magerl A thoracolumbar fractures without neurological symptoms than open surgery,with less iatrogenic injury,less intraoperative blood loss and quick recovery for patients.

ABSTRACT

Objective To investigate the effects of Liraglutide on plasma FGF-21 level in patients with type 2 diabetes mellitus (T2DM). Methods Enzyme linked immunosorbent assay (ELISA) was used to measure plasma FGF-21 level in patients with T2DM and normal controls (NGT). Relationship between plasma FGF-21level and BMI, WHR, HOMA-IR, HOMA-IS, FFA was analyzed. After Liraglutide treatment, plasma FGF-21level was observed in patients with T2DM. Results Fasting plasma FGF-21 level was higher in T2DM patients than that in normal control group ( 1.81 ±0.32 vs. 1.32 ±0.09 μg/L, P <0.01 ). After Liraglutide treatment, WHR, BMI, FPG, 2hPBG, HbA1c and FFA in T2DM patients significantly decreased ( P <0.05 or P <0. 01 ),HOMA-IS remarkably increased (P < 0. 01 ), and FGF-21 had no change. Conclusion Liraglutide and FGF-21may have different roles in insulin resistance.

ABSTRACT

An investigation of sphygmogram was carried out on 52 patients with middle type hypertensive syndrome and some normal women in late pregnancy were as the control. Their pregnancies were measured by a sphygmogram and the influence of pregnant hypertensive syndrome on the cardio-vas-cular system analyzed. It was found that left ventricular systolic force, cardial power and left ventricular effective work constant were stronger than those of the control. Meanwhile, myocardial energy invert constant decreased. This reflected the patients has compensative function in spite of increased myocardic load. In addition, pressure of peripheral arteries, blood volume of the patients, resistance and elasticmodulus of the artery were higher than those of the control, but capillary blood flow velocity decreased. Consequently, we have realized more about hemodynamic changes of hypertensive syndrome in pregnancy. And the authors consider that the sphygmogram may be used to detect changes of the cardio-vascular system of the patients with prcgancy toxemia and that the mean arterial pressure by a sphygmogram may reflect changes of hemodynamics better and earlier than a traditional sphygmomanometer. The sphygmogram may be used to make an early diagnosis of hypertensive syndrome.
